We will work with other health care providers to create a team centered care that is focused on you! As your care team, we will involve you in decisions about your health and health care to develop a stronger relationship with you. You will also have easier access to your provider through same day appointments and phone visits (when applicable).
It will take a partnership between you and us for the PCMH program to be successful. We will be discussing some changes at your next visit. If you have any questions please contact our staff. One of our providers is always available to our patients after hours by phone, for emergencies only. Please see the policy page for specifics.
